When Person in Charge was asked about the lack of towels in the toilet rooms, he replied that the food employees wash hands in the kitchen. Hands must be washed before leaving the toilet room.

Observed clean food equipment and/or utensils in prep area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Observed same violation on 3/6/20, 7/1/20 and 11/12/20.

2 – 102.11(A-C1)(C4-16) Person in Charge, Demonstration
The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

3 – 302.11 Packaged and Unpackaged Food-Separation, Packaging & Segregation
Several types of food in the walk-in cooler area are stored open with no covering. Same issue observed on 2/3/17 and 3/6/20.

4 – 601.11(A) Equipment, Food Contact Surfaces, Nonfood Contact Surfaces and Utensils
Metal food containers which are food contact surfaces, were observed to have food residue and was not clean to sight and touch. Same violation observed on 2/3/17, 3/6/20, and 7/1/20.

5 – 202.12 Handwashing Sinks, Installation
The handwash sink located in the front of the food prep area does not have water at a temperature of at least 100°F, because the hot water supply line valve was in the off position. Same violation observed on 3/6/20.

4 – 602.13 Nonfood-Contact Surfaces
Non-food contact surfaces (specifically the outside of a meat grinder) not cleaned at a frequency to preclude accumulation of dirt and soil. Same violation observed on 3/6/20 and 7/1/20.

Plumbing system not maintained in good repair – observed water leaking at the hot water supply line to the front prep area handwashing sink.

Faucet at the three-compartment sink is leaking and needs to be repaired. Most breakers in the circuit panel are not labeled. IPMC requires each breaker to be correctly labeled.
